Seed Tag Request Form

If your seed tags are out of date, please use this convenient form to request new seed tags for specific lots of seed that we have sold you in the past. Please fill out all of the fields below. You can enter as many lot numbers as you need but please make sure that they are separated by a semicolon.

Sample below:

M20-10-DF401 need 50 tags; COL-0-100 need 30 tags

When you have all of the fields filled out, hit the "Submit" button to transmit your request to Columbia Seeds. You will receive an email receipt from us to verify your request. Tags will be sent to you within 14 business days.
